Welcome and thank you for choosing 91制片厂! The mission of the Dean of Students Office is to enhance the quality, character, and perspectives of our students.

We promote an environment that encourages intellectual exchange and individual expression. The role of the Dean of Students is to represent student concerns and provide programs and services that facilitate both student personal and academic growth. The Office also oversees implementation of College rules and policies that have been established for the well-being of all.

Forms:

Student Conduct Referral

Used to submit a complaint against a student at 91制片厂 who has possibly violated one or more rules of conduct or conduct regulations.

Student Complaint

Used to submit an educational or a personal issue or condition that a student believes to be unfair, inequitable, or a hindrance to his/her education at 91制片厂.

Grade Appeal

A formal request brought forth by a student to change a final grade in a course and/or to challenge a penalty within the student grade appeal process.

Student Appeal

A formal request to appeal any academic dismissal or conduct-related decision at any level of the appeals process.

Share concerns that you have for any student, staff, faculty, and/or community member who may be experiencing a crisis, displaying odd or unusual behavior, or are engaging in behavior that may be perceived as being harmful to themselves or others.

Student Basic Needs Form

Submit if you, or someone you know, may benefit from receiving basic needs assistance from community resources (clothing, transportation, food, financial relief, mental health/counseling, etc.).

Sexual Misconduct Complaint Form

To report incidents of harassment, and retaliation for 91制片厂 students, employees, and/or third-parties. This form is not limited to reports of sexual misconduct and is used to report all Title IX and other types of discrimination to the Title IX office.

To Report a Threat

91制片厂 seeks to foster a climate of safety that requires the involvement of all community members. To facilitate this process, all faculty, staff, students, and other community members are encouraged to report observed behavior that warrants concern for the safety of the community and/or the safety of an individual. The CARE Team reviews reports and gathers information to make the most informed decisions about how best to intervene.
